Workers may know about practical problems that are absent from a written procedure. An audit can examine whether the organisation enables participation and responds to concerns without promising that certification eliminates harm.

Questions to follow in an audit

  • How can workers contribute to decisions affecting their work?
  • What happens after a concern or near miss is reported?
  • How are completed actions communicated back to the people affected?

Connect the records with practice

Trace one worker-raised concern through its assessment, response, action and the record showing how effectiveness was reviewed.

These are illustrative audit trails, not findings about a named organisation, technical instructions or a guarantee of a particular result. The appropriate evidence depends on the activities, risks and agreed scope. The organisation remains responsible for its operations and legal obligations.

AAA provides independent certification audits. Confirm the applicable standard edition, scope and customer requirements before agreeing a programme. Request an indicative price or discuss your proposed audit.
