An incident record is useful when it leads to understanding and effective action. An audit can examine the investigation and follow-up without assuming that a certificate prevents every injury or removes an employer's responsibilities.
Questions to follow in an audit
- Who decides the scope and competence needed for an investigation?
- How are worker accounts and relevant records considered?
- What evidence shows that the agreed action worked?
Connect the records with practice
Follow one incident or near miss from reporting through investigation, assigned action and a later effectiveness check.
These are illustrative audit trails, not findings about a named organisation, technical instructions or a guarantee of a particular result. The appropriate evidence depends on the activities, risks and agreed scope. The organisation remains responsible for its operations and legal obligations.
AAA provides independent certification audits. Confirm the applicable standard edition, scope and customer requirements before agreeing a programme. Request an indicative price or discuss your proposed audit.
