HSE's published 2024–25 fatality list records Thomas Eilbeck, aged 70, as having died following a fall from height on 12 April 2024 in Wigton. The earlier article incorrectly placed this incident in Tonbridge in April 2025. The HSE list is as-reported information, not an investigation finding, and does not establish the cause, equipment condition or certification status. Those unsupported assertions have been removed.

Questions to follow in an audit

  • Who checks the accuracy and limitations of incident information used by the organisation?
  • How are competent task assessment and worker communication arranged?
  • What evidence shows how reported concerns and changes are followed up?

Connect the records with practice

Trace one work-at-height activity through competent assessment, assigned responsibilities, worker communication and review of a change or concern.

These are illustrative audit trails, not findings about a named organisation, technical instructions or a guarantee of a particular result. The appropriate evidence depends on the activities, risks and agreed scope. The organisation remains responsible for its operations and legal obligations.

AAA provides independent certification audits. Confirm the applicable standard edition, scope and customer requirements before agreeing a programme. Request an indicative price or discuss your proposed audit.
